    اگرچه پژوهش های گوناگونی در حوزه طبابت در دوره های مختلف تاریخی ایران انجام پذیرفته، اما این پژوهش ها، بیشتر توصیفی هستند و به مسایلی کلی نظیر نام پزشکان، وظایف هر گروه، شیوه تحصیلات و... می پردازند. یکی از موضوعات مهمی که با وجود همه این پژوهش ها همچنان در حوزه فرهنگی ما بی جواب مانده، مفهوم و روش های طبابت در دوره های مختلف تاریخی است. در این مطالعه با استفاده از طبقه بندی میشل فوکو، فیلسوف فرانسوی که مطالعه گسترده ای را درباره مفهوم طب و تطور آن در قرن هجدهم اروپا انجام داده؛ سعی شده با بررسی متن های تاریخی، ابتدا مفهومی که طب در هر دوره تاریخی در گفتمان اجتماعی آن دوره (و در نتیجه در ذهن طبیب داشته) مشخص شود. سپس تفاوت این سیر تطور با آنچه فوکو در اروپای قرن هجدهم مشخص کرده نشان داده شده و در بخش بعدی به تفاوت بنیادین طب پیشامدرن با پزشکی مدرن پرداخته شده است. یافته های این پژوهش نشان می دهد که طب ایرانی که تا قرن های اولیه اسامی با تشریح کالبد آدمی موفق به شناختی درخشان از اعضاء و جوارح انسان شده بود؛ تلفیقی بوده است از «پزشکی مبتنی بر عوارض » و «پزشکی مبتنی بر طبقه بندی بیماری ». اما کم کم «طبقه بندی بیماری » به تدریج رنگ باخته و «پزشکی مبتنی بر عوارض » سهم بیشتری را از آن خود کرده است. رفته رفته، «پزشکی مبتنی بر عوارض » نیز به جای تکیه بر تجارب بالینی و کالبد شکافی، ابتدا روی به تکرار دانش گذشته و سپس در سیری انحطاطی به خرافات روی آورده است. همچنین نشان داده شده است که آنچه در قرن هجدهم اروپا، با کنار هم قرارگیری آموزش و درمان بیماری در نهادی به نام درمانگاه، به دلیل تغییر نگاه هستی شناختی به مفهوم کالبد انسان (یعنی ابژه شدن آن، چنان که در ذهنیت مدرن وجود دارد) حاصل شد و کالبدشکافی و پزشکی بالینی را سامان داد؛ در ایرانی که از قرن ها پیش از آن، کالبد شکافی درآن رایج بود و دو مورد بالا نیز در همان نهاد درمانگاه از پیش وجود داشت، به دلیل عدم تغییر این نگاه هستی شناختی، با چنین تکاملی همراه نبود و تنها به شناخت عوارض بیماری و همین طور به طبقه بندی انواع بیماری ها منجر شد.

    حفظ تندرستی و رهایی از رنج و بیماری یکی از مهم ترین مسایلی است که همواره در طول تاریخ، فکر بشر را به خود مشغول نموده. چنانچه با جستجو در آثار به جا مانده از پیشینیان، می توان از افکار و اندیشه های نیاکان خود در زمینه های مختلف آگاه شویم. ایرانیان از جمله اقوامی می باشند که دارای پیشینه ی تاریخی بسیار کهنند و از دوره ی باستان در زمینه های مختلف، مقررات خاص خود را داشته اند، از جمله این موضوعات، بهداشت می باشد. در پژوهش حاضر، سعی بر آن شده است که با روش توصیفی-تحلیلی و با اتکاء به منابع کتابخانه ای و تاریخی دست اول، پاسخی درخور برای سوال زیر بیابد: بهداشت و اصول بهداشتی در ایران باستان چگونه بوده است و روش های بهداشتی و درمانی ایرانیان باستان در مقابل بیماری های جامعه ی خویش به چه صورت می باشد؟ یافته های پژوهش حاکی از آن است که ایرانیان باستان به بهداشت توجه و اهمیت ویژه ای قایل بوده اند و برای بهداشت فردی و عمومی جامعه ی خود، مقررات و قوانینی خاص وضع کرده بودند و در منابع دینی و غیر دینی خویش، به دفعات بدان اشاره می کرده اند. در ایران باستان مردم اهمیت زیادی برای آب آشامیدنی سالم، دفع بهداشتی مدفوع، جداسازی بیماران، بهداشت فردی، لزوم نابودی حیوانات موذی و حشرات قایل بودند که نشانگر توجه آن ها به امر سلامت و پیش گیری از بیماری می باشد.

    در پژوهش حاضر به بررسی اطلاعات پزشکی و جایگاه علم پزشکی در ایران باستان و تاثیر بسزایی که دین در علم پزشکی ایران باستان داشته و همچنین به نقش مغان و موبدان به عنوان پزشکان اولیه ایران باستان با مراجعه به متون دینی تاریخی پرداخته می شود. در پزشکی ایران باستان، قوانین متقن پزشکی وجود داشته و علم پزشکی همگام با تمدن آن روزگاران، رشته های مختلف و دستاوردهای متفاوتی داشته است که از طریق جنگ ها و رفت و آمدهای ایرانیان و یونانیان به تمدن یونان منتقل و به نام آنها شهره گردید. پزشکی در ایران باستان برپایه پزشکی اوستایی بود که رد پای آ نرا م یتوان در برخی متون به جای ماند هی پهلوی دنبال کرد. علم پزشکی در این دوران، بالاخص در دوره ساسانیان هم چون سایر علوم، پیشرفت چشمگیری داشت. مطالب مربوط به پزشکی هخامنشیان به طور پراکنده در کتب دینی از جمله کتاب های سوم دینکرد، وندیداد و اوستا و مورخان یونانی آمده است.

    نگاهی سریع به مسایل مرتبط با شیوه هایی که مردم عصرصفویه، در مواجهه با بیماری های همه گیر به کار می بردند، ما را متوجه این حقیقت می سازد، که بشر از زمان پیدایش تا کنون، دغدغه ی درمان و غلبه بر بیماری ها را داشته است. لذا پژوهش حاضر براساس این مسئله شکل گرفته که مردم درجامعه ی ایران عصرصفوی برای رفع این معضل چه پیشگیری هایی را انجام می دادند و براساس چه شیوه هایی عمل می کردند و تبعات استفاده از این شیوه ها چگونه بوده است؟ با بررسی های انجام یافته، براساس روش توصیفی– تحلیلی مشخص گردید که به علت ناشناخته بودن و قابل پیش بینی نبودن این نوع بیماری ها، مردم دست به دامان معالجاتی می شدند که در دسترس بود و درمان گران، معالجات را با استفاده از فرهنگ و باورهای برگرفته از جامعه ی عصر صفویه انجام می دادند. اغلب درمان ها در عصر صفوی، یا بر پایه ی تفکر اسلامی و طب شرعی بوده و یا بر گرفته از طب سنتی، که مردم با استفاده از داروهای گیاهی و آمیزه ای از تجربه و باورهای عامیانه، آن را، انجام می دادند و یا این که با روی آوردن به معالجات ساحرانه و خرافی، با اعمال غیر منطقی، بیماری ها را درمان می کردند. بعضی از این معالجات، ناتوان از معالجه ی منشا اصلی بیماری بودند و در صورت ابتلا فقط به رفع نشانه ها و عوارض می پرداختند و تبعات ناشی از آن موجب طولانی تر شدن و ترس و حتی مرگ و میر درمیان مردم می شده است.

    یکی از مهمترین معضلات کرمانشاه در طول قرن نوزدهم، افزایش شیوع بیمارهای وگیردار و تلفات انسانی ناشی از آن بود. براساس بررسی ها، مرگ میر ناشی از این بیماری ها بیشتر به دلایل سبک زندگی ناصحیح و بی توجهی به بهداشت فردی بود. از دهه 1800 تا 1950 میلادی، کرمانشاه به دفعات درگیر اپیدمی های کشنده بود که تاثیر فراوانی بر روی سلامت و امنیت جامعه داشت. عدم پیشرفت دانش پزشکی و شرایط نامناسب بهداشتی در کرمانشاه؛ مانع انجام اقدامات لازم می شد و هر چند در دهه 1850 میلادی در زمان شیوع این بیماری ها، واکسیناسون و قرنطینه در برخی نقاط کرمانشاه به میزان اندکی انجام شده بود؛ اما تاثیر چندانی نداشت و شیوع بیماری های واگیردار مختلف باعث مرگ و میر و کاهش جمعیت کرمانشاه شد.

    جوامع از دیرباز به تخصصی به نام طبیب یا پزشک نیاز داشتند تا بیماری های آنها را درمان نماید. در ایران روش آموزش و تربیت طبیب، متون درسی و شیوه های معالجه، لااقل از زمان ابن سینا، یک نواخت و همسان بود اما در دوران قاجار و با نفوذ مدرنیته و علم جدید، طب نیز باید دگرگون می گردید اما این دگرگونی مانند هر تحول اجتماعی دیگری، به آسانی صورت نگرفت، مخالفان و موافقانی داشت و تا مدتی پهنه ی جامعه، صحنه ی جدال میان طب قدیم و جدید بود. این مقاله با مراجعه به منابع دست اول، تلاش دارد بخش هایی از این تحول را به تصویر کشیده و بیان دارد چگونه تا پایان عصر قاجار پزشکی جدید، با حمایت نهاد آموزشی دارالفنون و تصویب قوانین دولتی، توانست رقیب را ضعیف کرده و خود بر جای آن قرار گیرد. دو تحول انقلاب مشروطیت و روی کار آمدن سلسله پهلوی در تشدید این روند موثر بودند.

    روزنامه پرنفوذ اختر همواره در پی کیفی سازی نیروی انسانی و خدمات اجتماعی بود. در همین راستا، بهداشت نیز از جمله مسایل اجتماعی مورد توجه گردانندگان روزنام هی اختر بود. نویسندگان اختر، به طرق گوناگون به این موضوع می پرداختند و خوانندگان را به اهمیت بهداشت در ترقی و همچنین معضلات موجود بهداشتی آگاه می کردند. پرداختن به بهداشت در قالب های گوناگون انجام می گرفت. تشریح و اطلاع رسانی در معرفی انواع بیماری ها، اعلان های بهداشتی، تبلیغ داروهای جدید، آگهی های مربوط به پزشکان حاذق در درمان بیماری ها، شیوه های درمان بیماری ها و ذکر مضرات مواد مصرفی در بهداشت فردی و اجتماعی، از جمله موارد مورد تاکید نویسندگان اختر بود. استعمال مسکرات و نوشیدنی های الکلی و اثرات سوء بهداشتی نیز مورد توجه نویسندگان اختر بود. وجود مقالات متواتر در مضرات استفاده از مواد الکلی، حکایت از حساسیت گردانندگان اختر در این خصوص و تلاش آنان در آگاهی دادن به جامعه دارد. از منظر نویسندگان اختر سوء مصرف مواد الکلی، و عدم آگاهی علمی از مضرات آن نقش مهمی در تباهی نیروی انسانی، در ابعاد کیفی و اخلاقی داشت.

    وضعیت بهداشت و درمان مردم ایران، تا قبل از روی کار آمدن حکومت پهلوی، مطلوب و خوب نبود و مردم روستاها و شهرها از سلامت کافی برخوردار نبودند و حتی می توان ادعا کرد این وضع در روستا و شهرهای حاشیه ای ایران، به مراتب، نگران کننده تر بود. سطح سواد و آگاهی مردم نیز به گونه ای نبود که بتوانند کمکی به سلامت بهداشت جامعه خود بکنند. در آستانه ی شروع حکومت پهلوی اول، دولت با هدف پایه گذاری بهداشت عمومی مطلوب، یک سری استانداردهایی را در جامعه دنبال نمود تا در پی آن افزایش جمعیت اتفاق افتاده و بتواند در سال های آتی نیروی کار کافی تحویل کشور بدهد و شاخصه ی زاد و ولد را، به مرحله ی بالا رسانده و آن را کنترل کند. یکی از مواردی که باعث کاهش جمعیت ایران شده بود، وجود بیماری های واگیردار در زیر پوسته ی شهر و روستا بود. از جمله این بیماری ها، بیماری های مقاربتی همچون سفلیس و سوزاک بود، که می توان یکی از علل آن را پدیده ی روسپی گری و عدم نظارت کافی در امور بهداشتی و معضلات بهداشتی دانست. عدم درک صحیح نسبت به این مسئله و غفلت از توجه به آن، باعث شیوع بیماری دامن گیر سوزاک و سفلیس در بین مردم شد. در دوره ی پهلوی، در پی یک سری سیاست های بهداشتی، فعالیت هایی برای رسیدگی به وضع بیماری و رعایت بهداشت روسپیان و فواحش صورت گرفت. در این مقاله، سعی بر آ ناست نقش و عملکرد دولت در ارتقای بهداشت فواحش و جلوگیری از اشاعه ی بیماری سفلیس و سوزاک، با استفاده از اسناد و مدارک موجود، در مناطق مختلف ایران بررسی شود.

    بی تردید اسناد و مدارک تاریخی یکی از مهم ترین منابع برای تاریخ نگاری موثق و مورد اعتماد می باشد. هر یک از اسناد به لحاظ محتوایی می تواند مورد تحقیق جداگانه و مفصلی قرار بگیرند. در مورد اسناد تاریخی باید این نکته را متذکر شد که همواره این اسناد به عنوان شاهدان عینی تاریخ، در دادگاه عدل تاریخی حضور دارند. در همین رابطه، در این نوشتار برآنیم تا اسنادی چند از منطقه کمیجان را، از استان مرکزی، مورد بررسی محتوایی و ساختاری قرار دهیم. این اسناد درواقع عریضه های مردم کمیجان و فراهان به مجلس شورای ملی در سال 1305 شمسی است. در این اسناد مردم منطقه خواهان اعزام پزشک برای رسیدگی به اوضاع بهداشتی منطقه شده اند و از مجلس، تقاضای صدور اجازه نامه برای شخصی به نام «حمزه پزشکی » نموده اند. ازآنجاکه این اسناد در پیوند با تاریخ ایران دوره پهلوی است و گوش های از مشکلات اجتماعی منطقه را بررسی می کند، می تواند خود شاهدی برای بررسی اوضاع اجتماعی کشور در اوایل دوره ی پهلوی اول باشد. در طی بررسی این اسناد، ابتدا به معرفی کلی منطقه ی کمیجان و فراهان پرداخته و سپس، نگاهی بر تاریخچه بهداشت در ایران و از سویی تاثیر جنگ جهانی اول بر اوضاع بهداشتی ایران می کنیم و در ادامه، به بررسی سندشناسی اسناد و محتوای آنها می پردازیم.

    Although various researches has been done on medicine in different periods of Iran history, most of which are descriptive and deal with general problems such as physician’s names and the role of each group, way of educating and so on. Against all of these researches, one of the important matters that remained unanswered in our cultural field is the concept and methods of medicine in different period of history. Using the French philosopher Michel Foucault categories who has done wide studying on the concept and development of medicine in the eighteen century in Europe, by studying historical texts, in the paper, first it is attempted to define the concept of medicine in the social discourse of every period (as a result in the mind of physician). Then, the difference of the line of the development with what Foucault illustrates in the eighteen century Europe is presented. In the second part, the essential difference between pre modernism medicine and the modern one is clarified. Research findings show that the Iranian medicine which until the first Islamic centuries gained glorious knowledge about the human organs by anatomize human body, was a combination of medicine based on problems and medicine based on disease category, but gradually the categorizing of diseases disappeared and medicine based on problems attained more part. Step by step, medicine based on problems also instead of relying on clinical experiences and anatomy, first turn to previous knowledge and then going in a way of decline by paying attention to superstitious. In addition, it is clarified that what came about in Europe in the eighteen century by incorporating education and caring disease in the institution named clinic as a result of changing ontological vision to human anatomy ( namely, making it object, as in in the modern subjective) and regulated autopsy and clinical medicine, in Iran which in autopsy was common from previous centuries and the mentioned matters in the clinic institution, as a result no changing this ontological vision, such development did not happen a nd only resulted in knowing diseases problems and also to categorizing a variety of diseases.

    Maintaining health and getting rid of suffering and disease is one of the most important issues that has always occupied the human mind throughout history. Be aware of the different. Iranians are among the tribes that have a very ancient historical background and have had their own rules in various fields since ancient times, including health. In the present study, an attempt has been made to find a suitable answer to the following question by descriptive-analytical method and relying on first-hand library and historical sources: What was health and hygiene principles in ancient Iran and Iranian health methods? How does the ancients react to the ills of their community? The findings of the study indicate that the ancient Iranians paid special attention to health and set special rules and regulations for the individual and public health of their society and mentioned it many times in their religious and non-religious sources. They did. In ancient Iran, people attached great importance to safe drinking water, hygienic defecation, separation of patients, personal hygiene, the need to eradicate vermin and insects, which indicates their attention to health and disease prevention.

    In the present study, while examining medical information and the status of medical science in ancient Iran and the great impact that religion had on the medical science of ancient Iran, and also the role of Moghans and priests as early physicians of ancient Iran by referring to historical religious texts. In ancient Iranian medicine, there were strict medical laws and medical science, along with the civilization of those times, had different disciplines and different achievements that were transferred to the Greek civilization through the wars and travels of Iranians and Greeks and became famous by their names. In ancient Iran, it was based on Avestan medicine, the traces of which can be traced in some Pahlavi surviving texts, and on the other hand, they had relations with neighboring lands of Iran, including Egypt, Greece and Rome. Medical science in this period, especially During the Sassanid period, like other sciences, there was a significant progress. Materials related to Achaemenid medicine are scattered in religious books, including the third books of Dinkard, Vandidad, Avesta and Greek historians. But unfortunately, just as the Parthians lack information and resources in other cases, such reliable information can not be found in medical medicine.

    A quick look at issues related to the ways in which used by the people of the Safavid era in dealing with epidemic diseases brings us to the fact that human beings have been concerned with the treatment and overcoming of disease since its inception. Therefore, the present study is based on the question of what precautions did the people in the society of Iran in safavid era to solve this problem? And what methods did they use and what were the consequences of using these methods? Studies based on descriptive-analytical methods have shown that people resorted to the treatments that were available due to the unknown and unpredictable nature of these diseases and the therapists treated using beliefs and cultures derived from the Safavid era society. Most of the treatments in the Safavid era were either based on Islamic thought and Sharia medicine, or were based on traditional medicine which people performed using herbal medicines and a mixture of experience and beliefs or by turning to Magical and superstitious treatments treated the diseases with irrational practices. Some of these treatments were incapable of treating the main source of the disease and they only eliminated the symptoms and complications in the case of infection and the consequences resulted in prolonged fear and even death among the people.

    The outbreak of infectious diseases and human loses caused by which, was one of the most important problems of Kermanshah in the nineteen century. According to studies, the death caused by these diseases was often because of incorrect way of life and inattention to personal sanitation. From to the 1800s to 1950s, Kermanshah was affricated with deadly epidemics for several times which had many effects on the health and security of the society. Undeveloped medicine science and unsuitable medical condition of Kirmanshah prevented necessary actions and while in the 1800s in the time of spreading these diseases, vaccination and quarantine was done in some parts of Kermanshah, but culminated in little effects and the outbreak of infectious diseases caused death and much decrease in the population of Kermanshah.

    Communities have long needed a specialist called a doctor to treat their illness. In Iran, the method of education and training of physicians, textbooks and methods of treatment, at least since the time of Ibn Sina, were uniform. But during the Qajar period and with the influence of modernity and new science, medicine also had to change. But this transformation, like any other social transformation, did not take place as easily. There were pros and cons, and for a time the community was the scene of controversy between old and new medicine. This article tries to illustrate parts of this development by referring to first-hand sources and explains how by the end of the Qajar era, the new medicine, with the support of the Dar al-Fonun educational institution and the passage of government laws, was able to weaken the rival and replace it. The two developments of the Constitutional Revolution and the coming to power of the Pahlavi dynasty were effective in intensifying this process.

    Hygiene is one of the social issues that has been addressed by the Akhtar Newspaper. Akhtar, as an evolutionary newspaper, always sought to qualify manpower and social services in this direction. In a variety of ways, Akhtar’s authors focused on this issue and made readers aware of the importance of health in advancement as well as existing health problems. Hygiene was addressed in a variety of ways. Anatomy and information on the introduction of various diseases, health announcements, the promotion of new drugs, advertisements for physicians in the treatment of diseases, the methods of treating illnesses and mentioning the disadvantages of consumables in personal and social health were among the issues emphasized by the authors of Akhtar. The use of alcoholic beverages and alcoholic beverages and the health effects of this category of materials were considered by the authors of Akhtar. The results of the current research show that the existence of consistent articles on the harmfulness of the use of alcoholic substances indicates the actuality of the actuators in this regard and their efforts to inform the community. From the perspective of the authors of the drug abuse altogether, and the lack of scientific awareness of its disadvantages, it played a significant role in the deterioration of human resources in terms of quality and ethics.

    The health of the Iranian people was poor before the establishment of the Pahlavi government. The people of the villages and towns were not in good health, and in the villages and suburbs, the situation was far more worrying. With the beginning of the first Pahlavi regime, the government created laws in the society with the aim of creating public health in order to increase the population and provide sufficient labor force. One of the reasons for the decrease in Iran’s population was the existence of infectious diseases in cities and villages. These included sexually transmitted diseases such as syphilis and gonorrhea. One of the causes can be considered the phenomenon of prostitution and insufficient supervision in the health of prostitutes. Neglecting to pay attention to the health of prostitutes led to the spread of gonorrhea and syphilis .among the people In the Pahlavi period - with the enactment of some laws - work was done to address the disease and to observe the health of prostitutes and prostitutes. This article tries to examine the government’s performance in promoting the health of prostitutes and preventing the spread of syphilis and gonorrhea, using the documents available in Iran.

    Undoubtedly, historical documents are one of the most important sources for reliable and trustworthy historiography. Each of the documents in terms of content can be examined separately and in detail. In the case of historical documents, it should be noted that these documents are always present in the court of historical justice as objective witnesses to history. Let’s examine the content and structure of some documents from Komijan region of Markazi province. These documents are in fact petitions of Komijan and Farahan people to the National Assembly in 1305. In these documents, the people of the region have asked to send a doctor to take care of the region’s health. The parliament requested the issuance of a permit for a person named “Hamze pezeshki “. Since these documents are related to the history of Pahlavi Iran and examine some of the social problems of the region, it can be a witness to the social situation of the country in the early Pahlavi period. During the study of these documents first introduced the Komijan and Farahan region. And then we take a look at the history of health in Iran and the impact of World War I on Iran’s health, and then we look at the documentation of documents and their content.
