    زمینه و هدف

    سندرم تخمدان پلی کیستیک از مهم ترین دلایل اصلی ناباروری در خانم ها می باشد. هدف از انجام تحقیق حاضر تعیین تاثیر یک دوره تمرین تاباتا در آب و مصرف متفورمین بر E-selectin و NF-κB در زنان چاق مبتلا به PCOS  بود.

    روش کار

    جامعه آماری تحقیق نیمه تجربی و کاربردی حاضر را زنان دارای شاخص توده بدنی فراتر از (kg/m2) 9/29 شهر اصفهان که توسط متخصص غدد یا زنان و آزمایش های تشخیصی و سونوگرافی تشخیص سندرم تخمدان پلی کیستیک برای آن ها در نظر گرفته شده بود تشکیل دادند که از بین افراد داوطلب 30 نفر به عنوان نمونه آماری انتخاب و به طور تصادفی به دو گروه تجربی (متفورمین+تمرین تاباتا در آب) و کنترل تقسیم شدند. 24 ساعت قبل از شروع و 48 ساعت بعد از تمرینات خون گیری جهت اندازه گیری متغیرهای تحقیق انجام شد. سپس گروه تجربی به مدت 12 هفته، 3 جلسه در هفته و هر جلسه 40 دقیقه تمرینات تاباتا در آب را انجام دادند.

    یافته ها

    نتایج نشان داد تمرین در آب و متفورمین بر E-selectin (001/0=P) و NF-κB(025/0P=) در زنان چاق دارای سندرم تخمدان پلی کیستیک تاثیر معنی داری دارد.

    نتیجه گیری

    با توجه به تاثیر تمرین تاباتا در آب بر فاکتورهای التهابی و هسته ای استفاده از تمرین در آب و متفورمین برای بیماران با مشورت پزشک توصیه می شود.

    Background & Aims

    Overweight and obesity are related to increased risk of coronary heart disease, stroke, high blood pressure, type 2 diabetes and some specific cancers. In recent years, inactivity, obesity and metabolic syndrome have been increasing in developing societies (2). Among various diseases, polycystic ovary syndrome is among the cases that are strongly related to obesity (3).Studies in recent decades have shown that PCO syndrome can be a pro-inflammatory stage, because among them cardiovascular diseases, diabetes 2, insulin resistance and metabolic syndrome are seen with a high incidence (4). In the meantime, investigating and identifying the inflammatory causes of this condition for the above reasons can help specialists in its treatment with the process of reducing its negative effects through aerobic exercise. In this regard, the above research investigated some inflammatory factors such as E-selectin and nuclear factor kappa B (NF-κB).To improve the physical condition of obese people, instead of medicine, sports activities, both anaerobic and aerobic, are used, and each of these activities affects the improvement of physical condition through different mechanisms (2). Researchers believe that regular and not heavy exercise is a healthy and natural method for patients with polycystic ovary syndrome apart from clinical treatments (18). Physical activity improves menstrual irregularity, ovulation, ovarian morphology and fertility by reducing abdominal fat, blood sugar, blood fat, reducing insulin resistance, affecting hormone secretion and reducing cardiovascular risk factors. Be (7). But the question is, what exercise? With what intensity? And for how long?Treatment with all kinds of drugs may cause male characteristics, and unfortunately, most of the side effects are irreversible, so non-drug treatment strategies are necessary and need to be examined and studied. On the other hand, by reviewing the research done inside and outside the country, no research has been done on the effect of Tabata exercise on the research variables and even aerobic exercise on the research variables in women with polycystic ovary syndrome, so the purpose of this research is to determine The effect of a session of Tabata exercise in water and metformin on E-selectin and NF-κB in obese women with PCOS.

    Methods

    The current research was semi-experimental and applied with a pre-test-post-test research design. For this purpose, 30 women with a body mass index greater than 29.9 in Isfahan city, who were diagnosed with polycystic ovary syndrome by an endocrinologist or gynecologist and diagnostic tests and ultrasound, were considered as a statistical sample. They were selected and randomly divided into two experimental groups (metformin + exercise in water) and control. 24 Before starting the exercises, the subjects first completed the personal information questionnaire and blood was taken after 12 hours of overnight fasting to determine the level of the variables. Experimental group for 12 weeks, 3 sessions a week and each session 40 minutes of exercises in the water (including 10 minutes of walking forward, backward, sideways and soft running in the shallow part of the pool where the water level is below the neck and Then they performed stretching exercises. Then they performed Tabata exercise with a special exercise song played in the pool for 20 minutes, and then they did 10 minutes of stretching and cooling (22) and 500 mg metformin twice consumed during the day and after breakfast and dinner. 48 hours after the last training session, blood was taken again from all subjects. At the end, descriptive statistics (mean and standard deviation), Shapiro-Wilk test to check the normality of data distribution and covariance analysis to check research hypotheses using spss/21 software at a significance level of a≤0.05 were used. became.

    Results

    The results showed that the E-selectin factor in obese women with PCOS at the end of the period in the group of Tabata exercise in water and metformin intake had a significant difference compared to the control group (P=0.001 and F=134.6). Tukey's post hoc test results It showed that there is a significant difference between the pre-test of the Tabata training group in water and the post-test of the Tabata training group in water on the one hand, and between the post-test of the Tabata training group in water and the pre-test and post-test of the control group on the other hand, but between the other groups. There is no significant difference. (Table 2).The results showed that Tabata exercise in water and metformin consumption have a significant effect on NF-Κb factor in obese women with PCOS (P=0.025 and F=354.3). The results of Tukey's post hoc test showed that there is a significant difference between the pre-test of the Tabata group in water and the post-test of the Tabata group in water, but there is no significant difference between the other groups (Table 3).

    Conclusion

    The results showed that the E-selectin factor was significantly higher in obese women with polycystic ovary syndrome at the end of the period in the aerobic exercise and metformin group than in the control group. The results of the follow-up test showed that there is a significant difference between the pre-test of the Tabata aerobic training group and the post-test of the Tabata aerobic training group on the one hand, and between the post-test of the Tabata aerobic training group and the pre-test and post-test of the control group on the other hand, but between the other There is no significant difference between the groups. Aerobic exercises decreased VCAM-1 and ICAM-1 adhesive molecules; While its effects on E-selectin and P-selectin have been inconsistent. Of course, it seems that the effect of aerobic exercise on these conditions depends on the type and duration of the exercise intervention and the disease conditions such as the presence of ischemia. As presented in this review, there is a high level of evidence that sports activity has a positive effect on key factors in the development of atherosclerosis. This can partly explain the scientifically proven anti-atherogenic effects of aerobic exercise and has significant clinical effects (23).The results showed that the NF-Κb factor in obese women with polycystic ovary syndrome was significantly lower in the aerobic exercise and metformin group at the end of the period than in the control group. The results of Tukey's post hoc test showed that there is a significant difference between the pre-test of the Tabata aerobic exercise group and the post-test of the Tabata aerobic exercise group, but there is no significant difference between the other groups. In the explanation above, it can be stated that NF-κb is stimulated and activated in response to cellular stimuli. So far, about 450 stimuli, including physical, chemical, physiological and oxidative stimuli, have been identified for NFκB. Also, mitogens, receptor ligands, bacteria, viruses, parasites, fungi and their products, pro-inflammatory cytokines and some pathological conditions are among the stimuli of this transcription factor (28). The transcription factor NFκB is responsible for supplying a large number of pro-inflammatory genes, including cytokines, chemokines, immune receptors, enzymes and other pro-inflammatory molecules (29). Inappropriate activity of NFκB is one of the mechanisms of some diseases, especially those associated with inflammation (30).

    Background

     Breast and pelvic cancers are the most prevalent cancers among women globally. Several studies have reported on the effect of cell adhesion molecules on the growth, multiplication, invasion, and metastasis of tumor cells as well as inflammatory biomarkers, which are responsible for harmful inflammatory processes.

    Objectives

     The purpose of the current study was to assess the serum levels of adhesion molecules intercellular adhesion molecule-1 (ICAM-1), vascular cell adhesion molecule-1 (VCAM-1), and E-selectin in patients with cancer before and after radiotherapy, compare these with the levels of healthy subjects, and consider the relationship of these levels with the tumor origin.

    Methods

     The present case-control study investigated 14 patients with breast cancer and 14 patients with pelvic cancer who had been referred to Omid Teaching Hospital’s oncology clinic, Mashhad, Iran between 2015 and 2017. Evaluated by the ELISA method for ICAM-1, VCAM-1, and E-selectin were the serum samples of these 28 cancer patients before and after their course of radiotherapy treatment and the serum samples of the 28 healthy subjects who had no history of cancer, radiotherapy or the risk factor of coronary artery disease.

    Results

     The ICAM-1, VCAM-1, and E-selectin serum levels of all patients with cancer before and after a course of radiotherapy were significantly higher than those of the control group (P-value ≤ 0.5). There was no significant difference between the two cancer groups before and after radiotherapy (P-value ≥ 0.05).

    Conclusions

     The current study demonstrated that the serum levels of adhesion molecules in patients with cancer before and after radiotherapy increase significantly regardless of the initial location of the tumor.

    مقدمه

    زنان مبتلا به سندرم تخمدان پلی کیستیک (PCOS) در معرض خطر افزایش بیماری های قلبی- عروقی هستند. تمرین ورزشی یک راهکار درمانی برای بهبود عملکرد عروقی و کاهش میزان مرگ و میر در بیماران مختلف است. از این رو پژوهش حاضر با هدف بررسی اثر 8 هفته تمرین هوازی تناوبی شدید (HIIT) بر برخی عوامل عملکرد اندوتلیال عروقی؛ شامل VCAM-1 و E-selectin، در مبتلایان به این سندرم انجام شد.

    مواد و روش ها

    سی زن مبتلا به PCOS با سن 18 تا 40 سال، به طور تصادفی به 2 گروه شاهد و HIIT تقسیم شدند. جلسات تمرین به مدت 8  هفته، سه بار در هفته اجرا شد. قبل و پس از مداخله؛ مقادیر سرمی VCAM-1 و E-selectin، پس از 10 ساعت ناشتایی شبانه، در هر دو گروه اندازه گیری شد. داده های گردآوری شده با استفاده از نرم افزار SPSS نسخه 23 تحلیل شدند.

    یافته ها

    در گروه تمرین، اختلاف آماری معنی دار بین مقادیر سرمی E-selectin، قبل و بعد از مداخله وجود داشت (P=0/037)، در حالی که در گروه شاهد قبل و بعد از مداخله، اختلاف آماری معنی دار مشاهده نشد (0/05<P). هم چنین، در هیچ یک از گروه ها، اختلاف آماری معنی دار بین سطوح VCAM-1، قبل و بعد از مداخله وجود نداشت (0/05<P).

    نتیجه  گیری

    یافته های این مطالعه نشان داد که 8 هفته تمرینات HIIT می تواند با کاهش سطح سرمی E-selectin، در بهبود عملکرد اندوتلیال عروقی در زنان مبتلا به PCOS موثر باشد.

    Introduction

    Women with polycystic ovary syndrome (PCOS) are at increased risk of developing cardiovascular diseases. Exercise is a therapeutic strategy to improve vascular function and reduce patient mortality. Thus, this study aimed to evaluate the effect of eight weeks of high-intensity interval training (HIIT) on serum levels of VCAM-1 and E-selectins in women with PCOS.

    Materials and Methods

    Thirty females with PCOS aged between 18 and 40 years were randomly categorized into an HIIT group and a control group. The HIIT program was administered three sessions a week for eight weeks. Serum levels of VCAM-1 and E-selectins were measured before and after the intervention after an overnight fast of 10 h. The data were analyzed using SPSS software (version 23).

    Results

    After the intervention, a significant difference was observed in serum E-selectin levels in the HIIT group (p=0.037) compared to the control group (P>0.05). However, no significant changes were observed in serum VCAM-1 levels in the HIIT group after the intervention (P≥0.05).

    Conclusion

    The findings demonstrated that eight weeks of HIIT could improve endothelial function in women with PCOS.

    زمینه و هدف  :

    مولکول های چسبان، نشانگرهای سلولی شناسایی روند تشکیل پلاک آترواسکلروزی دیواره اندوتلیال عروق هستند. هدف، بررسی تغییرات سرمی E-selectin و P-selectin در مردان مبتلا به نارسایی قلبی، پس از هشت هفته تمرین هوازی تداومی و تناوبی بود.

    مواد و روش ها:

     42 مرد با نارسایی قلبی، نمونه گیری در دسترس انتخاب، به سه گروه مساوی (14 نفر) تمرین هوازی تداومی، تناوبی و کنترل تقسیم شدند. تمرین تداومی (شدت 45 تا 70 درصد ضربان قلب بیشینه) و هوازی تناوبی (شدت 45 تا 80 درصد ضربان قلب بیشینه با استراحت 5 تا 10 دقیقه ای بین هر تناوب) هشت هفته (سه روز در هفته) بود. نمونه خونی، قبل و پس از هشت هفته از آزمودنی ها گرفته و متغیرها به روش الایزا اندازه گیری شدند. داده ها با تحلیل کوواریانس، سطح معنی داری 05/0≥P تحلیل شدند.

    یافته ها:

     مقایسه بین گروه ها در سطوح E-selectin  در هر سه گروه، تفاوت معنی داری نشان داد. آزمون بنفرونی، در E-selectin تفاوت معنی داری بین گروه هوازی تناوبی و کنترل (0/005=P) و بین هوازی تداومی و کنترل (0/006=P) و P-selectin تفاوت معنی داری بین گروه هوازی تناوبی و کنترل (0/032=P) نشان داد ولی بین گروه هوازی تداومی و کنترل (0/083=P)، تغییر معنی داری مشاهده نشد. تفاوت معنی داری در متغیر های مورد مطالعه در دو گروه هوازی تناوبی و تداومی مشاهده نشد.

    نتیجه گیری :

    تمرین هوازی با کاهش سطوح مولکول های چسبان، احتمالا نقش موثری در پیشگیری و کنترل بیماری های قلبی- عروقی مردانی که مبتلا به نارسایی قلبی هستند دارد.

    Introduction

    Adhesive molecules are cellular markers that identify the process of atherosclerotic plaque formation in the endothelial wall of arteries. The aim was evaluation of changes in E-selectin and P-selectin in men with heart failure after eight weeks of continuous and intermittent aerobic exercise.

    Materials and Methods

    42 men with heart failure were selected by sampling method as volunteers and were divided into three equal groups (14 subjects) continuous aerobic, intermittent aerobic exercises and control groups. Continuous training (45-70% of Maximum Heart Rate) and intermittent training (45-80% of MHR with 5-10 minutes rest between each period) were done for 8 weeks (three days a week). Fasting blood samples were taken from all subjects before and after  eight weeks of aerobic exercise and adhesion molecules were measured by ELISA method. Data were analyzed by ANCOVA at significance levels of P<0.05.

    Results

    Comparison of the groups with covariance analysis showed that the levels of E-selectin (P=0.002) and P-selectin (P=0.022) levels was significant in all three groups. The Bonferroni test showed, There was a significant difference between the intermittent and control (P=0.005) and between continuous and control groups (P=0.006) at the level of Eselectin and the intermittent aerobic and control groups (P=0.032) at Pselectin level, However, there was no significant change despite a decrease between the control and continuing aerobic (P=0.083) in Pselecteine. Also, there was no significant difference in the studied variables in both intermittent and continuous aerobic groups.

    Conclusion

    Aerobic exercise with reducing levels of adhesion molecules may play an important role in the prevention and control of cardiovascular diseases in men with heart failure.

    Thyroid dysfunction is associated with elevated cardiovascular risk factors and atherosclerosis. It could be suggested that, hyperthyroidism is related to a higher prevalence of arterial abnormalities. Therefore, evaluating the endothelial dysfunction (ED) related biomarkers seem to be an important issue. It is not clear whether endothelial cells are biologically responsive to thyroid hormones (THs) or how THs induces the production of endothelial cells (EC)-derived proinflammatory mediators. Hence, in this study the effects of thyroxine (T4) on ED and inflammatory related mediators were evaluated. Human umbilical vein endothelial cells was used as endothelial cell model which was treated with concentrations of 50, 100, 200 nmol/L of T4 in various exposure times. In the following, gene and protein expression levels of EC-related markers including intercellular adhesion molecule-1 (ICAM-1), vascular endothelial growth factor (VEGF), and E-selectin were determined using real time polymerase chain reaction (RT-PCR) and western blotting methods. Also, interleukin-6 (IL-6) and tumor necrosis factor (TNF-α) protein levels as proinflammatory cytokines were determined by enzyme linked immunosorbent assay (ELISA) method. Gene and protein expression analysis revealed that T4 treatments up regulated the levels of VEGF, ICAM-1, and E-selectin as ED markers. In addition, T4-treated cells had higher significant levels of IL-6 and TNF-α versus untreated cells in different incubation times. This study proposed the atherosclerotic effects of thyroid hormone. Based on our findings, T4 had strong effects on the gene and protein expression levels of pro-inflammatory, angiogenesis, and ED major mediators associated with atherosclerosis development.

    Background

    Alopecia Areata (AA) is an immune system disease that invades anagen Hair Follicles (HFs) and shows multifactorial etiology. One of the essential histopathological features of AA is a lymphocytic intrusion that envelops HFs. Soluble forms of Endothelial (E), Lymphocytic (L), and Platelets (P)-selectins are known markers of persistent bothering. There are limited examinations for the assessment of serum levels of soluble E-selectin, L-selectin, and P-selectin in patients with AA.

    Objectives

    We assessed serum levels of soluble E-selectin and L-selectin in patients with in association with the control group to determine their role in the pathogenesis of the disease.

    Methods

    The study included 30 patients with AA and 10 healthy subjects who served as controls. Blood samples were taken from all patients and controls. The serum levels of soluble E-selectin and L-selectin were determined with the Enzyme-linked Immunosorbent Assay (ELISA) method.

    Results

    Higher levels of sE-selectin and sL-selectin were found in AA patients than in controls. There was a significant relationship between the higher serum levels of sE-selectin and the increased size of lesions with time. There was a significant positive correlation between the serum levels of sE-selectin and sL-selectin and the percentage of hair loss.

    Conclusions

    This study showed that sE-selectin and sL-selectin may play a significant role in the pathogenesis of AA and might open a way to future treatments for this disease

    Atherosclerosis is a multifactorial disorder, which affects the arterial wall. It has been reported that, hypothyroidism and thyroid hormone deficiency are related to cardiovascular disorders. Also, endothelial dysfunction plays an essential role in the development of atherosclerosis. We aimed to evaluate the effects of thyroid-stimulating hormone (TSH) on pro-inflammatory tumor necrosis factor-α (TNF-α), interleukin-6 (IL-6), angiogenic vascular endothelial growth factor (VEGF) and leukocyte adhesion, intercellular adhesion molecule 1 (ICAM-1) and E-selectin in human umbilical vein endothelial cells (HUVECs). In this study, HUVEC cells were treated with 1 and 2 µM of TSH in different treatment times. The gene and protein expression of ICAM-1, VEGF, and E-selectin were performed by real-time polymerase chain reaction and western blotting, respectively. Likewise, TNF-α and IL-6 protein levels were determined by the ELISA method. VEGF, ICAM-1, and E-selectin as endothelial dysfunction markers and also, TNF-α and IL-6 as pro-inflammatory cytokines were detectable in HUVEC. Besides, the results of this study revealed that TSH treatment down-regulates TNF-α and IL-6. Evaluating the gene and protein expression data revealed the upregulation of ICAM-1, E-selectin, and VEGF in TSH treated cases in different periods of exposure. Considering the multiple actions of TSH, it could be concluded that TSH plays a controversial role in atherogenesis by anti-inflammatory effects and on the other side, angiogenesis and leukocyte adhesion induction which is related to vascular cell proliferation.

    Background- Selectins as a group of adhesion molecule arise from the surface of activated cell and involved for atherosclerotic plaque production.Objective- To evaluate the relationship between Circulating Endothelial-Leukocyte adhesion molecule -1 (E-selectin) and the extent of coronary artery involvement in male Patients with coronary heart disease (CHD).Methods- Participants were 103 male patients with symptoms of CHD. Patients underwent coronary angiography and according to angiography results they were divided to 80 patients with severe CHD and 23 with mild coronary involvements according to angiographic score. The Circulating E-selectin level was compared in both groups by ELISA method. Anova and analysis of covariant (ANCOVA) were used for evaluating possible association of E-selectin level and severity of coronary artery involvement by adjusting age, history of diabetes mellitus, hypertension, and dyslipidemia.Results- The mean age of 103 male patients was 58.32±9.42 years. The E-selectin level was not significantly different in both groups with sever and minimal (CHD) (P=0.78). ANCOVA test could not show after significant association between E-selectin level and the extent of coronary artery involvement adjusting age (P=0.94) as well as history of diabetes mellitus, hypertension, and dyslepidemia respectively (P=0.43).Conclusion- The circulating E-selectin level have no association with the severity of CHD in male patients.
