سندروم درد کشککی-رانی (Patellofemoral Pain Syndrome; PFPS) یک آسیب شایع در دوندگان و افراد فعال محسوب می شود. هدف از این تحقیق بررسی تفاوت در تغییرپذیری هماهنگی و عملکرد عضلانی اندام تحتانی در ورزشکاران مبتلا به PFPS و ورزشکاران سالم حین دویدن بود.
روش بررسیدر این تحقیق 18 ورزشکار مرد دچار PFPS و 18 ورزشکار مرد سالم شرکت کردند. رکوردگیری با کمک 6 حسگر واحد اندازه گیری لختی (Inertial Measurement Unit: IMU) که بر روی ران ، ساق و پای دو طرف و همچنین الکترودهای سطحی که بر روی عضلات پهن داخلی، پهن خارجی و سرینی میانی قرار گرفته بودند انجام شد. تغییرپذیری هماهنگی و فعالیت الکتریکی اندام تحتانی هر شرکت کننده حین دویدن با سرعت خودانتخابی به مدت 21 دقیقه روی تردمیل در ابتدای تست حین فاز استقرار و نوسان و در انتهای تست فقط حین فاز استقرار مورد بررسی قرار گرفت. برای تجزیه و تحلیل آماری از آزمون t مستقل استفاده شد (0/05≥p).
یافته هاتغییرپذیری هماهنگی ساجیتال ران-ساجیتال ساق ، ساجیتال ران-عرضی ساق، عرضی ران-عرضی ساق، فرونتال ران-عرضی ساق، ساجیتال ساق-عرضی پا و عرضی ساق- ساجیتال پا در گروه PFPS نسبت به گروه سالم حین فاز استقرار (0/05≥p) و ساجیتال ساق-عرضی پا حین فاز نوسان (0/001=p) کمتر بود. زمان فعالیت عضلات پهن داخلی و سرینی میانی گروه PFPS نشان داد این عضلات نسبت به گروه سالم دیرتر فعال شدند (p=0/005 و p=0/002) و برای مدت طولانی تری (p=0/007 و p=0/001) با فعالیت کمتر فعال باقی ماندند (0/001=p).
نتیجه گیریبا توجه به پژوهش دریافتیم که ضعف عملکرد عضلات پهن داخلی و سرینی میانی می تواند یکی از دلایل کاهش تغییرپذیری هماهنگی در اندام تحتانی باشد.

Patellofemoral Pain Syndrome (PFPS) is a common ailment among runners and those leading active lifestyles. This study aimed to explore the differences in the coordination variability and muscle functionality in the lower limbs of professional athletes afflicted with PFPS compared to their healthy counterparts during running.
MethodsEighteen male athletes with Patellofemoral Pain Syndrome (PFPS) and 18 healthy male athletes participated in this study. Data recording was conducted using 6 Inertial Measurement Unit (IMU) sensors placed on both thighs, shins, and feet, along with surface electrodes on the vastus medialis, vastus lateralis, and gluteus medius muscles. The coordination variability and electrical activity of the lower limbs of each participant were examined while running at a self-selected speed on a treadmill for 21 minutes, during both the stance and swing phases at the test's beginning, and only during the stance phase at the test's end. An independent t-test was utilized for statistical analysis (p≥0.05).
ResultsCoordination variability between sagittal thigh-sagittal shin, sagittal thigh-transverse shin, transverse thigh-transverse shin, frontal thigh-transverse shin, sagittal shin-transverse foot, and transverse shin-sagittal foot was lower in the PFPS group compared to the healthy group during the stance phase (p≥0.05), and sagittal shin-transverse foot coordination variability was lower during the swing phase (p=0.001). The activation timing of the vastus medialis and gluteus medius muscles in the PFPS group was delayed compared to the healthy group (p=0.005 and p=0.002, respectively) and remained active for a longer duration (p=0.007 and p=0.001, respectively), with less overall activity (p=0.001).
ConclusionThe research indicates that the performance of the muscles studied may be a contributing factor to the reduced coordination variability observed in the lower limbs.

مقدمه و اهداف
الگوهای بارگذاری هرمی و هرمی معکوس در تمرینات مقاومتی به منظور افزایش بار تمرین استفاده می شود. مطالعات فیزیولوژیکی سعی در مقایسه این دو الگو کرده اند، بااین حال تفاوت این دو الگوی بارگذاری بر بیومکانیک ستون فقرات و لگن مشخص نشده است. مطالعه حاضر با هدف بررسی تاثیر دو نوع بارگذاری هرمی و هرمی معکوس بر تغییرپذیری هماهنگی ستون فقرات و لگن در حرکت اسکوات بود.
مواد و روش هادر این مطالعه 12 فرد ورزشکار پرورش اندام شهر تهران به طور داوطلبانه شرکت کردند. آزمودنی ها حرکت اسکوات را به صورت هرمی و هرمی معکوس در شدت های 50، 70 و 90 درصد با 10 تکرار بیشینه انجام دادند و اطلاعات کینماتیکی سه اندام ستون فقرات و لگن آن ها ثبت شد. هماهنگی و تغییرپذیری هماهنگی با استفاده از روش وکتورکدینگ اصلاح شده محاسبه شد.
یافته هانتایج تحلیل واریانس با اندازه گیری تکراری نشان داد برای هماهنگی لگن به پایین کمر و برای هماهنگی پایین کمر به ستون فقرات تحتانی سینه ای و ستون فقرات تحتانی سینه ای به ستون فقرات فوقانی سینه ای تفاوت معناداری در الگوی هماهنگی در شدت 50 درصد مشاهده شد و برای هماهنگی کمر به ستون فقرات تحتانی سینه ای تفاوت معناداری در الگوی هماهنگی در شدت 70 درصد مشاهده شد. همچنین برای تغییرپذیری هماهنگی ستون فقرات و لگن هیچ گونه معناداری آماری بین دو نوع بارگذاری هرمی و هرمی معکوس مشاهده نشد.
نتیجه گیریطبق نتایج افزایش بار در بارگذاری هرمی و کاهش بار در بارگذاری معکوس، هماهنگی کوپلینگ های دیستال را بیشتر از پروگزیمال در صفحه ساجیتال تحت تاثیر قرار داد. توصیه می شود از روش هرمی معکوس برای انجام تمرینات استفاده شود و در انجام تمرینات هرمی بیشتر احتیاط شود.

Although physiological studies have attempted to compare the two patterns, the difference between the two loading patterns on the biomechanics of the spine and pelvis has not been determined. This study aimed to investigate the effect of pyramid and inverted pyramid loadings on spinal and pelvic coordination variability in squat movement.
MethodsTwelve bodybuilding athletes in Tehran City, Iran, voluntarily participated in this study. The subjects performed squat movements as the pyramid and reverse pyramid at intensities of 50%, 70%, and 90% of ten maximum repetitions. Then, the kinematic information of the three segments of their spine and pelvis was recorded. Coordination and variability coordination were calculated using a modified vector coding method.
ResultsThe results of repeated measures analysis of variance showed a significant difference in the coordination pattern for pelvic-lower back coordination, lower-back to lower-trunk spine, and lower-trunk-to-upper-trunk spine in the intensity of 50%. A significant difference was observed in the coordination pattern, with an intensity of 70% for the lower back and lower trunk spine. Also, no statistical significance was observed between the pyramidal and inverted pyramidal loading types for spine and pelvis coordination.
ConclusionAccording to the results of increasing the load in pyramid loading and decreasing the load in reverse loading, the coordination of distal couplings affected the sagittal plane more than the proximal one. It is recommended to use the reverse pyramid method for exercises and be more careful when performing pyramid exercises.

مقدمه
راهکار مناسب برای کاهش اثرات منفی لیفت بر کمردرد، توجه محققان را به خود جلب کرده است. یکی از استراتژیهای احتمالی که در برابر اثرات ناشی از بارهای وارد شده به ستون فقرات استفاده میشود، کمربندهای وزنهبرداری میباشد. هدف از انجام پژوهش حاضر، بررسی تاثیر کمربندهای وزنهبرداری بر تغییرپذیری هماهنگی ستون فقرات و لگن در لیفت تکراری ورزشکاران مرد مبتلا به کمردرد مزمن غیر اختصاصی بود.
مواد و روشها12 ورزشکار مبتلا به کمردرد مزمن به صورت اختیاری در این مطالعه شرکت کردند. آزمودنیها حرکت لیفت یک جعبه را با فرکانس 10 بار در دقیقه به مدت 1 دقیقه با و بدون کمربند انجام دادند و اطلاعات کینماتیکی بر اساس مدل سه سگمنتی ستون فقرات با استفاده از سیستم موشن کپچر (Motion Capture) مجهز به 10 دوربین ثبت گردید. هماهنگی و تغییرپذیری هماهنگی این سگمنتها با استفاده از روش وکتور کدینگ اصلاح شده محاسبه شد. به منظور مقایسه دادههای جمعآوری شده در صورت نرمال بودن، از آزمون Paired t در نرمافزار SPSS و در دادههای تغییرپذیری از روش نقشهبرداری پارامتری آمار (Statistical parametric mapping یا SPM) در نرمافزار Matlab با سطح معنیداری 05/0 > P استفاده گردید.
یافتههابرای هماهنگی سگمنت لگن به سگمنت پایین کمر، تفاوت معنیداری در الگوی هماهنگی مشاهده شد [فاز کانسنتریک (035/0 = P) و فاز اکسنتریک (043/0 = P)]. همچنین، هیچگونه تفاوت معنیداری بین استفاده و عدم استفاده از کمربند برای هماهنگی سگمنت کمر به سگمنت تحتانی تنه (545/0 = P)، سگمنت تحتانی تنه به سگمنت فوقانی تنه (440/0 = P) و تغییرپذیری تمام کوپلهای محاسبه شده وجود نداشت.
نتیجهگیریبا توجه به یافتهها، شاید استفاده از کمربند باعث شود انتقال حرکت از سگمنت دیستال به پروگزیمال به خوبی صورت گیرد. اگر این فرضیه برقرار باشد، میتواند فشار بر روی کمر را کاهش دهد و باعث جلوگیری از آسیب دیدگی و درد در ناحیه کمر گردد. بنابراین، به ورزشکاران دارای کمردرد توصیه میشود که در هنگام لیفت بارهای سنگین و به منظور این که انتقال حرکت از سگمنت دیستال به پروگزیمال به طور صحیح صورت گیرد، از کمربند استفاده نمایند.

One of the practical strategies to deal with the effects of loads on the spine is lumbar belts. The aim of this study was to investigate the effect of lumbar belts on the coordination variability of spine and pelvis during repetitive lifts in male athletes with non-specific chronic LBP.
Materials and MethodsTwelve male athletes with chronic LBP participated in the study voluntarily. Participants with and without belts lifted the box for one minute at a frequency of 10 times per minute, and cinematic information based on the three-segment model of spine was recorded using a motion capture system equipped with 10 cameras. Coordination and coordination changes for these segments were calculated using a modified vector coding (VC) method. To compare the collected data, if normal, paired t-test was used in SPSS software and in variability data, statistical parametric mapping (SPM) method was used in MATLAB software with the significance level of P ≤ 0.05.
ResultsA significant difference was observed for the coordination of the pelvic segment to the lower back segment [concentric phase (P ≤ 0.035), eccentric phase (P ≤ 0.043)]. On the other hand, no significant difference was reported between using and not using belts for the coordination of the lumbar segment to the lower trunk segment (P = 0.545), lower trunk segment to the upper trunk segment (P = 0.440), and the variability of all calculated couples
ConclusionAccording to the findings, using the belt may optimize the transfer of motion from the distal segment to the proximal segment, which can reduce the pressure on the waist and prevent injury and pain in the lumbar region. Therefore, it is recommended that athletes with LBP use a belt when lifting heavy loads in order to transfer movement from the distal to the proximal segment correctly.

مقدمه
هدف از انجام پژوهش حاضر، مقایسه تغییرپذیری هماهنگی سگمنت های اندام تحتانی در مردان دارای زانوی پرانتزی و سالم حین دویدن روی تردمیل با سرعت های مختلف بود.
مواد و روش ها15دانشجوی سالم رشته علوم ورزشی و 15 داوطلب دارای ناهنجاری زانوی پرانتزی در این مطالعه شرکت کردند. هر آزمودنی با سرعت ترجیحی، 25 درصد سریع تر و 25 درصد آهسته تر از سرعت ترجیحی، به مدت 60 ثانیه روی تردمیل دوید و در حین دویدن، اطلاعات کینماتیکی اندام تحتانی آن ها توسط سیستم آنالیز حرکت مایوموشن با فرکانس نمونه برداری 200 هرتز ثبت گردید. محاسبات هماهنگی با استفاده از روش فاز نسبی پیوسته برای سه کوپلینگ سگمنتی انجام و بین دو گروه مقایسه شد.
یافته هانتایج آزمون (Repeated measures ANOVA) بین گروهی نشان داد که تفاوت معنی داری بین الگوهای هماهنگی سگمنت ها در دو گروه حین دویدن با سرعت های مختلف وجود نداشت(P > 0/05)، اما تغییر سرعت دویدن، منجر به بروز تفاوت معنی داری در هماهنگی و تغییرپذیری هماهنگی سگمنت ها در برخی فازهای دویدن در هر دو گروه شد (P < 0/05).
نتیجه گیریالگوی هماهنگی سگمنت های مورد بررسی در حین دویدن بین دو گروه دارای زانوی پرانتزی و سالم متفاوت نمی باشد، اما تفاوت های معنی داری در این الگوها و سرعت های متفاوت دویدن در هر دو گروه مشاهده شد. از طرف دیگر، تغییرپذیری هماهنگی سگمنت ها بین دو گروه متفاوت نبود، اما افزایش سرعت دویدن، باعث کاهش تغییرپذیری هماهنگی در ابتدا و انتهای سویینگ و میداستنس در هر دو گروه شد که می تواند افزایش احتمال بروز صدمات پرکاری در این فازها را نشان دهد.

Materials and MethodsAmong sports and physical education students, 15 healthy men and 15 men with genu varum voluntarily participated in this research. Each participant ran on treadmill for 60 seconds at preferred speed and 25% faster and 25% slower than preferred speed. The kinematics data of lower extremities were collected by MyoMotion motion analysis system at 200 Hz sampling frequency. Coordination calculations were done by continuous relative phase (CRP) method for three segments coupling and compared between two groups.
ResultsThe results of repeated measures analysis of variance (ANOVA) between groups showed that coordination patterns of segments did not differ significantly between the groups during running at different speeds (P ˃ 0.05). However, the change in running speed caused significant differences in coordination and coordination variability of segments in some phases of running in two groups (P ≤ 0.05).
ConclusionCoordination pattern of segments during running seems the same between genu varum and healthy groups; however, significant differences in these patterns and different running speeds were evident in both groups. On the other hand, coordination variability of segments was not different between groups, but the increase in running speed reduced the coordination variability during early and late swing and mid-stance phase of running in both groups. This finding may imply higher risk of overuse injuries at these phases.
