    Background

    The objective of this study was to evaluate the lips Morphology in Iranian adults with normal occlusion in Isfahan, Iran.

    Materials and Methods

    This cross-sectional study analyzed the lateral cephalograms of 102 individuals aged 18 to 30 years with normal occlusion in Isfahan, Iran. Nine linear measurements (upper lip to S line (ULS), lower lip to S line (LLS), upper lip to B line (ULB), lower lip to B line (LLB), upper lip to E line (ULE), lower lip to E line (LLE), lower lip to H line (LLH), and upper and lower lip thickness), and 5 angu-lar measurements (nasolabial, inferior labial, Z, upper lip prominence and lower lip prominence angles) were made on the lateral cephalograms. The study compared these measurements between males and fe-males using the Mann-Whitney and independent t tests (α=0.05).

    Results

    Assessment of lips morphology using 9 linear and 5 angular measurements showed variances in the mean values of several measurements compared to standard values reported by Ricketts, Steiner, Burstone, and Merrifield. Specifically, differences were observed in the ULE, LLE, ULS, LLS, ULB, and Z angle, while the LLB, LLH distances, and the nasolabial angle fell within the standard range. Additionally, males exhibited significantly greater upper and lower lip thickness compared to females (P<0.001). No other sig-nificant differences were found between males and females (P>0.05).

    Conclusion

    Most of the measured indices in the study population differed from the standard values report-ed in the literature, which highlights the need to take into account the Iranian norms in orthodontic treat-ment of Iranian patients.

    Introduction

    Secretory carcinoma, previously known as mammary analog secretory carcinoma, is a rare malignancy of salivary glands. It has a diversity of microscopic patterns and is similar to other salivary gland tumors.

     Case Report:

    This report presents the case of a 32-year-old female patient with a painless swelling of the upper lip and a history of recent surgery for an immature ovarian teratoma.  The microscopic sections revealed a circumscribed neoplasm composed of macrocystic, papillary-cystic, and microcystic patterns with bland vesicular nuclei and vacuolated cytoplasm. Tumoral cells were strongly positive for mammaglobin, SOX10, GATA3, S-100, and vimentin. The diagnosis of salivary gland secretory carcinoma was made. After 22 months, there has been no recurrence. 

    Conclusions

    As secretory carcinoma is a relatively new entity, it is necessary to understand its characteristics. Although the overall incidence of second primary cancer in patients with salivary gland cancers is low, the possibility of its presence in such patients should be considered.

    مقدمه

     خطوط لب در افراد منحصر به فرد و با گذر سن پایدار است و می توان از آن برای تشخیص جنسیت فرد در دندان پزشکی قانونی استفاده نمود. هدف این مطالعه، ارزیابی خطوط لب در افراد و بررسی امکان تعیین جنسیت از طریق خطوط لب بود.

    مواد و روش ها

     در این مطالعه ی توصیفی- تحلیلی، 100 نفر شامل 57 زن و 43 مرد از دانشگاه آزاد اسلامی اصفهان در اردیبهشت ماه 1397 شرکت نمودند. با استفاده از رژ لب قرمز مارک نوبا، شماره 405 ساخت کشور ایران روی نوار چسب 50 میلی متری و چسباندن آن بر روی کاغذ سفید اثر لب تهیه گردید. هر اثر لب به چهار بخش تقسیم شد و به طور مستقل توسط دو ناظر با استفاده از یک لنز دوربین، شیارهای لب ثبت شد. برای تعریف الگوهای لب، از روش طبقه بندی سوزوکی و تسوشی هاشی استفاده گردید. تحلیل داده ها با آزمون های Pearson، Chi-square، Fisher’s exact test، Kolmogorov-Smirnov و Independent sample T-test انجام شد (0/05 = α).

    یافته ها

     یافته های این مطالعه نشان داد که هیچ گونه مشابهتی در خطوط لب افراد وجود نداشت. بیشترین فراوانی از نوع الگوی e (31 درصد) و پس از آن به ترتیب نوع a (30 درصد)، نوع c (21 درصد)، نوع d (8 درصد)، نوع b (6 درصد) و نوع f (4 درصد) بود. در تفکیک لب به چهار ناحیه، در ناحیه چپ لب فوقانی بین زنان و مردان تفاوت معنی داری وجود داشت (0/004 = p value)، الگوی e در مردان و الگوی c در زنان بیشترین فراوانی را داشت. همچنین از نظر ارتفاع لب و کشیدگی لب تفاوت معنی داری بین زنان و مردان دیده نشد.

    نتیجه گیری

     خطوط لب منحصر به فرد است و می تواند در کنار سایر روش ها، در تشخیص جنسیت مورد استفاده قرار گیرد و در ناحیه ی چپ لب فوقانی الگوی e بیشتر در مردان و الگوی c بیشتر در زنان بروز می کند.

    Introduction

    Lip lines in individuals are unique and stable with age and can be used for sex estimation in forensic dentistry. This study aims to evaluate lip lines in people and investigate the possibility of sex estimation through lip lines.

    Materials & Methods

    In this descriptive-analytical study, 100 people including 57 females and 43 males from Isfahan Islamic Azad University participated in May 2017. By using Nuba brand red lipstick, number 405, made in Iran, on 50 mm adhesive tape and pasting it on white paper, a lip effect was prepared. Each lip was divided into four parts, and the lip grooves were recorded independently by two observers using a camera lens. Suzuki and Tsuchihashi’s classification methods were used to define lip patterns. Data analysis was performed using Chi-2 Pearson, Fisher's exact, Kolmogorov Smirnov, and independent t-tests (α = 0.05).

    Results

    Our study showed that there was no similarity in people's lip lines. The highest frequency was pattern type e (31%) and then type a (30%), type c (21%), type d (8%), type b (6%), and type f (4%) respectively. In dividing the lip into four areas, there was a significant difference between females and males in the left upper lip area (P=0.004), pattern e in males and pattern c in females were the most frequent. Also, there was no significant difference between females and males in terms of lip height and lip elongation.

    Conclusion

    Lip lines are unique and can be used in sex estimation. In the left upper lip area, pattern e is more common in males, and pattern c is more common in females.

    Background

    Cheiloscopy is a forensic investigation technique based on the identification of lip traces. This study sought to assess the distribution of lip print patterns and their stability during the time.

    Methods

    Lip prints were obtained from 133 dental students and the distribution of patterns was evaluated, using the Five-stage Suzuki and Tsuchuhashi’s classification. Six months later, lip prints were obtained again from 40 out of 133 students who were chosen accidentally to assess the stability. The individual lip formula was determined by Paired T-test and ANOVA at p<0.05. 

    Results

    Type I was the most common pattern in the lips. Lip formulas were found to be unique for each individual. No significant difference existed in the distribution of lip print patterns between males and females, or between the upper and lower lips. In terms of stability, 73.8% of lip print patterns remained unchanged during a 6-month period. 

    Conclusion

    Lip print patterns are unique to each individual and have medium stability over time. Therefore, cheiloscopy may be considered a useful identification tool in forensic dentistry. However, the lip print pattern cannot indicate the sex of people.

    کیست های موکوسی احتباسی متعدد ضایعات نسبتا نادری بویژه در لب پایین هستند . در این گزارش مورد درباره مردی 60 ساله با تورم متعدد در مخاط باکال راست و چپ و مخاط لبیال پایین، می باشد. بیوپسی انجام شد و ارزیابی بافت شناسی اتساع مجاری غدد بزاقی فرعی و تشکیل کیست های متعدد مشاهده شد. فاکتورهای متعددی مانند تغییر در ترشحات بزاقی، نقص مادرزادی و یا اکتسابی در ساختار مجاری ممکن است با این رخداد ناشایع ارتباط داشته باشد. تنگ شدن مجاری به علت استفاده از دهانشویه های حاوی هیدروژن پراکسید، خمیر دندان های خوشبوکننده، ضد پلاک و ضد جرم نیز پیشنهاد شده اند.

    Multiple mucous retention cysts are relatively rare conditions, particularly on the lower lip. This case report presents a 60-year-old man with multiple swelling structures located in the right and left buccal mucosa, and lower labial mucosa. A biopsy was done, and a histologic assessment confirmed dilation of minor salivary ducts and cystic formation. Several factors may interfere with the creation of this uncommon phenomenon, such as alteration in salivary secretion, and congenital or acquired weakness in the ductal structure. In addition, it is suggested that ductal narrowing may be due to the long-term application of some mouthwash containing hydrogen peroxide, deodorant also anti-plaque, and tartar-control toothpaste.

    Introduction

    The lip repositioning surgical technique arose with the objective of correcting and harmonizing the act of smiling. Since the conventional technique was published, some modifications of the technique have been proposed in order to counteract postoperative recurrence and to achieve the best esthetic appearance of the smile. The objective of this paper was to describe the laser-assisted lip repositioning technique (laser-assisted LRS) with a 940nm diode laser and 2780nm Er, Cr: YSGG, as a modification to the conventional lip repositioning technique, for the treatment of a gummy smile.

    Case Report

    The proposed technique consists in achieving the descent of the upper lip by removing the intraoral mucosal band through the laser peeling of the oral mucosa, preserving the connective tissue intact for healing by secondary intention. The technique was implemented in two patients with a diagnosis of a gummy smile caused by a short and hyperactive upper lip; the healing process was satisfactory, and there was no bleeding or postoperative edema. Postoperative controls were performed at 3 and 10 months, and no recurrence was found.

    Conclusion

    the 940nm Diode laser or 2780nm Er, Cr: YSGG laser-assisted lip repositioning the technique described here is less invasive than that performed with a conventional scalpel, offers aesthetic results, lower risk of infection, and recurrence of the gummy smile in the long term.

    Objectives

    This study investigated the prevalence and pattern of lip premalignant lesions in patients referred to the Cancer Institute of Imam Khomeini and Shohada-E-Tajrish Hospitals between 2004 and 2016.

    Methods

    This retrospective cross-sectional study was conducted on the pathology reports of patients retrieved from the archives of the Pathology Departments of the Cancer Institute of Imam Khomeini and Shohada-E-Tajrish Hospitals between 2004 and 2016. The gender, age, lesion location (upper lip, lower lip, commissure, lip in general, not stated), pathological type, and clinical diagnosis of the lesions were extracted from patient records. The Fisher's Exact test was used to analyze the data by SPSS version 16.

    Results

    Of a total of 237,392 patients, 40 (0.02%) cases had lip premalignant lesions. The mean age of patients was 63.71±14.11 years (range 3 to 92 years). The prevalence of lip premalignant lesions was higher in males, with a male to female ratio of 4:1. The most common location and histopathological type of lesions were the lower lip, and actinic keratosis (60% of the cases), respectively.

    Conclusion 

    Lip premalignant lesions were observed in 0.02% of patients. Although this rate is lower than the global prevalence of precancerous lesions of the lip and oral cavity (4.47%), because of the high malignant transformation rate of lip premalignant lesions, every clinician must take part in early detection of these lesions by clinical examination. To confirm the diagnosis, biopsy may be requested for histopathological diagnosis.

    For many years oral esthetic problems treated by dentists were limited to those involving the teeth without giving consideration to the gingiva. However, today it is well established that these structures should be in balance to appear esthetically pleasing. More than 3mm gingival show during smiling is considered as ‘excessive gingival display’ also known as “gummy smile”, which is not attractive. The current case series, presents three patients with gummy smile managed by lip repositioning surgery, each with 1 year follow up. Our objective was to introduce lip repositioning as a successful treatment modality to decrease gingival show using a simple and conservative surgical approach.

    BACKGROUND

    Augmentation of facial components is an acceptable approach in facial aesthetics. The best filler material and the optimal technique for facial soft tissue augmentation still remain to be determined. This study has assessed the lip augmentation using post-auricular fibroareolar tissue as a new autogenous filler.

    METHODS

    This prospective study enrolled patients who were candidate for lip augmentation. Loose fibroareolar tissue was harvested from post-auricular region and was inserted in the prepared lip pocket. Standard digital photography was used for lip analysis in each patient before and 6 months after surgery (the height and projection of the upper lip and lower lip).

    RESULTS

    At a mean follow-up period of 6 months, average upper and lower lip height increased from 5.27 to 8.72 mm (p<0.001), and the average upper and lower lip projection increased from 3.97 to 7.75 mm (p<0.001).

    CONCLUSION

    Our technique was minimally invasive and a safe method, in which post-auricular loose fibroareolar tissue was applied for lip augmentation with minimum donor site morbidity and long-term results. On the follow-up, all patients were satisfied with the results and no patients required any revision operation. Moreover, the complication rate was negligible.

    Introduction

    Gastrointestinal Stromal Tumor (GIST) is an uncommon mesenchymal tumor of the gastrointestinal tract. It is rarely presented as Multiple Primary Malignancy Syndrome (MPMS) without an association with the familial syndromes.

    Case Presentation

    A 63-year-old Iranian Azeri man admitted to a government hospital, affiliated to the Tabriz University of Medical Sciences, Tabriz, Iran, in 2012. The illness presented with complicated gastrointestinal problems with the diagnosis of GIST of the stomach, treated with imatinib. They family history and the screening tests of malignancies were negative, but the patient diagnosed with Squamous Cell Carcinoma (SCC) of the lower lip, leading to 28 courses of radiotherapy, followed by the recurrence of gastrointestinal symptoms, the GIST and SCC of the esophagus, that was also detected accidentally.

    Conclusions

    Management of the multiple primary malignancy syndrome needs multidisciplinary teamwork. The study of MPMs, its etiology, and epidemiology can provide us proper ways of management.

    Schwannoma is one of the most common benign intracranial tumors which originates from the Schwann cells of neural sheaths in the upper part of vestibular branch of the eighth nerve, especially in patients with neurofibromatosis type 2. Schwannoma in the other parts of head and neck region is, however, relatively uncommon.The present report describes a rare case of schwannoma that was accidentally discovered in the upper lip in asymptomatic patient lasting for more than 10 years with recently size enlargement. Schwannoma could be seen in a variety of anatomic sites. Thus, this diagnosis should be considered in the differential diagnosis of soft tissue tumor in any location.

    Statement of the problem: Nowadays leishmaniasis is a common infectious disease around the world which is caused by a protozoan parasite named Leishmania. This parasite is transmitted to human by an infected female sand fly. This disease has three clinical forms; mucosal forms are rare in Iran.
    Purpose
    The purpose of this study was to report a case of leishmaniasis with lip and oral mucosal involvement that healed successfully.
    Results
    This article is about patients with lip leishmaniasis with oral-mucosal involvement, clinical features were diffuse lip swelling and ulcer in right lower lip vermilion and buccal mucosa. A smear was caught which proved the diagnosis of leishmaniasis. Thereafter, the patients were referred to a dermatologist for treatment and follow up showed complete healing of the lesions.
    Conclusion
    Oral mucosal leishmaniasis is rare. However, the swelling may cause different complications like loss of teeth or respiratory obstruction. It should be noted that time is a key point in treatment. So, it is necessary for us to train the differential diagnosis of chronic ulcerated oral lesions to our dentists.

    Hemangioma, which is a rife benign proliferative lesion with vascular tissue origin, is the most common vascular anomaly of infancy. Cellular hemangioma is one type of these tumors, known as the vascular channel formation of endothelial cells. This report describes a 34-year old man with an exophytic lesion on the lower lip , firm, dome shape ,smooth surface with mild keratosis who came to us two months ago ; histopathologic finding showed a relatively rare subtype of hemangioma, cellular hemangioma ,which characterized by the potential for recurrence and cellular immaturity . Among hemangioma subtypes, the most common are cavernous and capillary hemangiomas; a relatively rare subtype is cellular hemangioma, characterized by the potential for recurrence and cellular immaturity. CH most commonly involved parotid gland, but may also occur internally in the oral mucosa in infancy and early childhood. Mmucocele and pyogenic granuloma were considered in the differential diagnosis for our case.

    هدف
    این مطالعه با هدف بررسی عوامل خطرساز مرتبط با شکاف لب و کام در گروهی از نوزادان شهر تهران از سال 1391 تا 94 انجام شد
    مواد و روش ها
    در این مطالعه مورد شاهدی 105 مورد از نوزادان مبتلا به شکاف های دهانی و 218 نوزاد غیرمبتلا و مادرانشان جهت بررسی عوامل مرتبط با بیماری، در بیمارستان مفید شهر تهران مورد بررسی قرار گرفتند. داده ها با آزمون کای دو آنالیز شد
    یافته ها
    از 105 بیمار، 40 نفر مونث (%38) و 65 نفر مذکر (%62) بودند. فراوانی شکاف لب و کام %58، شکاف کام به تنهایی 29 نفر (%27/6) و شکاف منفرد لب 15 نفر (%14/2) بود. بین ازدواج فامیلی و شکاف دهانی ارتباط آماری معنی داری وجود داشت (0/001=P). %4/51 از نوزادان دارای شکاف، مادران با سن 25 تا 35 سال داشتند. 43 نفر (%41) از مادران گروه مورد تاریخچه مصرف سیگار را دارا بودند که تفاوت معنی داری بین دو گروه دارای شکاف و فاقد شکاف وجود داشت (P =0/001)
    نتیجه گیری
    در این مطالعه ازدواج فامیلی، سن بالاتر از 35 سال مادر و سیگار کشیدن مادر در دوران بارداری افزایش خطر ایجاد شکاف را به ترتیب حدود 18، 17 و 14 برابر نشان می دهد و این موضوع اهمیت مشاوره قبل از ازدواج، قبل از بارداری و تغییر شیوه زندگی قابل اصلاح را نشان می دهد تا به این طریق خطر تولد نوزادان مبتلا به شکاف های صورتی کاهش یابد

    Background
    Cleft lip and palate is one of the most common congenital anomalies. The etiology of non syndromic orofacial clefts is multifactorial.
    Objective
    The aim of this study was to evaluate the related risk factors of cleft lip and palate in a group of infants born in Tehran during the years between 2012-2015.
    Methods
    In this case-control study the files of 105 newborns with oral clefts, and 218 normal newborns as control group with their mothers were evaluated in Mofid Hospital in Tehran. Data were analyzed by Chi-square test.
    Findings: Among 105 cases, 40 cases (38%) were females and 65 cases (62%) were males. The frequency of cleft lip and palate, cleft palate and cleft lip were 58%, 27.6% and 14.2% respectively. A significant relationship was found between parents with familial marriage (P=0.001). The highest number of clefts belonged to 25-35 year-old mothers (51.4%), 41% of mothers reported smoking during pregnancy thus maternal smoking would be an effective predisposing factor to have a child with oral clefts (P=0.001).
    Conclusion
    The results of this study indicate that history of familial matrimony, mother’s age above 35 years and maternal smoking during pregnancy can enhance the risk of orofacial clefts 18, 17 and 14 times respectively. These findings emphasize the importance of preconception counseling of mothers-to-be on amendable lifestyle factors in order to reduce the birth prevalence of cleft lip/ palate in future generations.

    Introduction
    Leishmaniasis is a protozoal disease transmitted by sand-fly. The term leishmaniasis encompasses multiple clinical syndromes.
    Case Presentation
    A 24-year-old male native of Yazd, Iran, was referred from the dental and subsequently to the infectious clinic with complaints of ulcer and swelling over his lower lip for three months. The lesion progressively increased in size over a few weeks and there was no history of fever or other complaints. The lesion had response to treatment with multiple antibiotics by various medical practitioners.
    Conclusions
    The case was interesting according to the rare location of the lesion since it can be confused with other infectious diseases.

    Background
    Excessive gingival display (GD) is a frequent finding that can occur because of various intraoral or extraoral etiologies. This work describes the use of a mucosal repositioned flap for the management of a gummy smile associated with vertical maxillary excess (VME) and hypermobility of the upper lip followed by injection of Botox.
    Materials And Methods
    Seven female patients in the age range of 17–25 years presented with a gummy smile. At full smile, the average GD ranged from 6 to 8 mm. A clinical examination revealed hypermobility of the upper lip. A cephalometric analysis pointed to the presence of VME. The mucosal repositioned flap surgery was conducted followed by injection with botulinum toxin type A (Botox) 2 weeks postsurgically.
    Results
    After 4 weeks, results were definitely observed with a decrease from 8 mm gingival exposure to 3 mm, which was considered as normal GD for an adult during smiling.
    Conclusion
    For patients desiring a less invasive alternative to orthognathic surgery, the mucosal repositioned flap is a viable alternative. Moreover, Botox is a useful adjunct to enhance the esthetics and improve patient satisfaction where surgery alone may prove inadequately in moderate VME.

    Approximately 25% of all oral cavity carcinomas involve the lips, and the primary management of these lesions is complete surgical resection. Loss of tissue in the lips after resection is treated with a variety of techniques, depending on the extension and location of the defect.Here we review highly accepted techniques of lip reconstruction and some of new trials with significant clinical results. Reconstruction choice is primarily depend to size of the defect, localization of defect, elasticity of tissues. But patient’s age, comorbidities, and motivation are also important. According to the defect location and size, different reconstruction methods can be used. For defects involved less than 30% of lips, primary closures are sufficient. In defects with 35–70% lip involvement, the Karapandzic, Abbe, Estlander, McGregor or Gillies’ fan flaps or their modifications can be used. When lip remaining tissues are insufficient, cheek tissue can be used in Webster and Bernard advancement flaps and their various modifications. Deltopectoral or radial forearm free flaps can be options for large defects of the lip extending to the Jaws. To achieve best functional and esthetic results, surgeons should be able to choose most appropriate reconstruction method. Considering defect's size and location, patient's expects and surgeon's ability and knowledge, a variety of flaps are presented in order to reconstruct defects resulted from tumor ablation. It's necessary for surgeons to trace the recent innovations in lip reconstruction to offer best choices to patients.

    Introduction
    Exfoliative Cheilitis (EC) is a rare chronic inflammatory condition affecting the vermillion border of the lips and characterized by excessive production of keratin. In this paper, a case of chronic EC that was relatively resistant to usual treatment is to be introduced. Also, we explain its clinical course and treatment procedures and measures.
    Case Presentation
    We report a 25-year-old Iranian woman presented with desquamation of her lips. History, clinical feature and histopathological examination revealed a diagnosis of EC.
    Conclusions
    Although treatment of EC in the present study had considerable success in the short-term follow-up, it should be noted, due to the unknown of the exact cause, no specific treatment or protocol has still been identified..

    Introduction
    Anthropometry (morphometry of the human) is measurement of linear and angular dimensions of the human body. Since the anthropometric parameters which are based on age, sex, geographical location and ethnic characteristics of the human species are different, therefore, anthropometric studies of population by age and sex should be done separately.
    Methods
    In this study, anthropometric measurements lips and different patterns of lip line were studied in 133 males (4-40 years) with Persian ethnic who were living in Mashhad. These persons had no inflammation, herpes, malformations such as cleft lip, and surgery history in lib and jaw. Lip full-face photos were taken using a digital camera (Nikon, 12.3 Mega Pixels). Anthropometric measurements of mouth width (ch-ch); width of philtrum (cph-cph); height of the total lip ((ls-li;) the entire lip area (total area) were measured on computer by MIP software. Data were evaluated statistically using SPSS, polynomial regression and ANOVA models (P<0.001).
    Results
    There was a significant correlation between age and parameters of the mouth width, lips height to nose and total area of the lip. Conclussion: Considering significant relationship between the quantities investigated and age, we can use anthropometric data of lips in many fields including medical, criminal, etc. By comparing this study with other studies, it can be concluded that there is the most similarity in the size of lip of Persian men in Mashhad with men living in India. It is hoped that in the similar future studies, extensive statistical societies be prepared in each races of Turks, Baluchis, Kurds in order to complete anthropometric database of lip in Iranian Persian ethnics.
